ruv 7f02c87c6f test(server): add multi-node mesh integration tests (ADR-068) ...

8 tests covering per-node state pipeline:
- Frame builder validity (CSI + vitals packet formats)
- Different nodes produce different I/Q patterns
- Multi-node UDP send (1/3/5/7/11 nodes)
- Mesh simulation with variable rates and node dropout
- Large mesh: 100 nodes x 10 frames = 1,000 frames
- Max scale: 255 unique node_ids

All 26 server tests pass (8 new + 18 existing vital signs).
